Each year, the IEP team will meet to develop the Individualized Education Plan for your child. The IEP team generally consists of the parent(s), a Special Education teacher/Case Manager, an LEA (which stands for Local Educational Agency. This role is usually filled by the Principal or another administrator), a General Education Teacher (if your child receives services within the general education classroom), and any supportive service staff such as Speech Therapist or Occupational Therapist. The following are different sections of an IEP. Click on the link to read more about each section.

Parent's Guide to Understanding Rights and Responsibilities - This document contains your rights as a parent as it pertains to your child's education and services within the special education program. This particular document comes from the Georgia Department of Education, as that is where I am located, but other states most likely have their own versions of this document as well.
One way to prepare yourself as a parent/guardian for your child's IEP meeting is to create an IEP binder to organize information you will receive at the meeting as well as any communication from your child's teachers throughout the school year. The link below shares tips for organization and includes printable and a video.
